ZAP® (Zero Acceptance of Pain)

ZAP is a cancer pain assessment program designed to improve the treatment of cancer pain by improving communication between patients and their health care providers. The ZAP program aims to help patients, caregivers, and physicians understand that cancer pain treatment is an important part of the overall good cancer care. ZAP was developed in collaboration with cancer specialists and includes tools to help patients and health care professionals record, assess, and manage cancer pain.

All of the following documents require the availability of the Acrobat Reader. If you haven’t installed the Acrobat reader on your system, you can download it free of any charge.

The ZAP documents are:

ZAP - Pain Assessment and Management Guide

This is a reference guide for professionals to help them obtain a complete patient pain assessement. It includes a standard pain assessment scale and a list of key questions that professionals can ask patients to properly assess cancer pain.

ZAP - Patient Self-Assessment Form

This form helps patients record different aspects of their pain (frequency, duration, intensity, etc.). Patients can complete this form and take it to their physician or nurse at each scheduled office visit.

ZAP - Patient Self-Report Journal

This journal helps patients become more aware of their pain experience. It provides a series of questions to help patients write down their observations when they experience pain.
